Lymphogranuloma Venereum (LGV) is a sexually transmitted disease (STD) caused by specific strains of the Chlamydia trachomatis bacteria. Although once considered rare in industrialized countries, LGV has been increasingly reported in regions such as Singapore, especially within high-risk groups. Early detection and treatment are critical to avoid complications.

What is Lymphogranuloma Venereum (LGV)?

LGV is caused by the L1, L2, and L3 serovars of Chlamydia trachomatis, which are more invasive than the strains that cause common chlamydial infections. It primarily affects the lymphatic system, rectum, and genital regions.

How Does LGV Differ from Other Chlamydia Infections?

Unlike typical Chlamydia infections, which are often mild and limited to the genital area, LGV is more severe and can cause systemic damage. LGV is also unique because it spreads to the lymphatic system, leading to painful inflammation and long-term complications if untreated.

Symptoms of Lymphogranuloma Venereum (LGV) in Men and Women

LGV progresses through three distinct stages, with symptoms varying slightly between men and women:

First Stage (3 days to 1 month after exposure):

Small, painless ulcers appear on the genitals or rectal area. These may go unnoticed as they are often asymptomatic.

Men may notice these small genital sores more easily.

Women might miss the first signs, especially if ulcers develop internally.

Second Stage (2 to 6 weeks after exposure):

Men: The most common symptom is a painful swelling of the lymph nodes in the groin (inguinal lymph nodes). These swollen nodes, called buboes, can rupture and discharge pus if untreated.

Women: In addition to swollen groin lymph nodes, women may experience back pain, pelvic pain, and painful swelling of deeper lymph nodes near the cervix or vagina, which can lead to vaginal discharge and discomfort.

Both men and women may develop systemic symptoms like fever, joint pain, and fatigue.

Third Stage (Months to years after infection):

If left untreated, LGV can cause rectal ulcers, bloody discharge, rectal pain, and a feeling of incomplete bowel movements, especially in people who engage in receptive anal intercourse.

Women may experience additional complications like vaginal scarring and pelvic inflammatory disease, which can lead to long-term damage.

Both sexes can suffer from chronic swelling of genital tissues due to lymph node obstruction, causing deformity and discomfort.

How is LGV Transmitted?

LGV is spread through sexual contact, including vaginal, anal, and oral ex. It can also be transmitted through the sharing of contaminated sex toys. Direct contact with infected sores or bodily fluids increases the risk of transmission.

Testing for LGV in Singapore

Testing for LGV involves:

Swab tests of genital or rectal ulcers to detect the Chlamydia trachomatis bacteria.

Urine tests can also help identify the presence of the bacteria.

It is important to seek testing early, as LGV can mimic other sexually transmitted infections like herpes or syphilis. Learn more: STD Testing in Singapore

Treatment for Lymphogranuloma Venereum (LGV)

LGV is treatable with antibiotics. Completing the entire course of treatment is essential to ensure the infection is fully cleared and to prevent complications.

Sexual partners of LGV patients should also be tested and treated, even if they have no symptoms.

Prevention of LGV

To prevent LGV, practice safe sex by:

Using condoms during all sexual activities

Regularly testing for STDs if you have new or multiple sexual partners

Avoiding the sharing of sex toys unless properly sanitized or covered with new condoms

Lymphogranuloma Venereum (LGV) is a serious but treatable STI. If you experience any symptoms described above or are at high risk, it is important to seek testing and treatment. Early intervention is crucial to avoid long-term complications such as rectal or genital scarring, especially for those engaging in high-risk sexual behaviours.
